File of Life 

"Be prepared for that unexpected emergency!"

In many cases, medical emergencies are not expected, leaving the patient and their families not prepared. A high stress situation such as a medical emergency can also cloud your memory causing you to forget basic information. 

This is why Lewis County Fire District 6 offers the "File of Life" program free of charge to it's citizens.

The "File of Life" is a form to be filled out with medical information, allergies, medications, etc. and has a protective plastic cover with a magnet for fridge placement. In addition to filling out the informational form, we encourage residents to also include advance directives signed by the patient and their physician.
